At a minimum, we need you to fill out the Short Application In Order To Start The Pre-Approval Process. Don't worry if there are a few blanks that don't apply. Fill out as much as possible and when we speak, we will make sure we have all the information needed.

 How To Get A Duplicate Copy Of Form DD214
 
Your DD214 form is required in order to obtain your certificate of eligibility for a VA mortgage loan.  If you have misplaced your original DD214 form you can request another copy by completing and submitting Form SF 180 "Request Pertaining To Military Records."  You can download Form SF 180 by Clicking Here.

 
You can also request dd214 duplicate copies by using the VA fax on demand system.  Just dial 301-837-0990 from a fax machine. Follow the voice instructions and when prompted for the document number enter 2255.

The DD-214 can now be requested online, but ONLY BY:
  • A military veteran, or
  • Next of kin of a deceased, former member of the military.

The next of kin can be any of the following: surviving spouse that has not remarried, father, mother, son, daughter, sister, or brother. Persons fitting one of these two categories can request military records online from the eVetRecs Website by Clicking Here.

Be sure to read all instructions before beginning the request process.

Costs and Response Time

"Generally there is no charge for military personnel and health record information provided to veterans, next-of-kin, and authorized representatives. If your request involves a service fee, you will be notified as soon as that determination is made. Response time varies dependent upon the complexity of your request, the availability of records, and our workload. Please do not send a follow-up request before 90 days have elapsed as it may cause further delays."

Texas Vet Forms

You will find 3 pages in this form package. Once you print these 3 pages, please note where you are to sign. Please fill them out and fax them to me. Note: Texas Vet is only looking for your last 12 months of address info, including your APO (if you are active and out of the country). One of the forms requires that a Notary witness your signature. If you can get it notarized, great. If not, don't worry, just photocopy and fax your Drivers License or Military ID, and we will handle it on our end.

VA Form 26-1880
This form is extremely important to us gaining your Certificate of Eligibility from the VA. We will send this form to the VA Administration, and within a few weeks you will receive a Certificate of Eligibility (green colored) in the mail from the VA. We will need the original from you.
VA Form DD 2058

In order to gain your eligibility to use the Texas Vet Program, they require that your LES reads "TX" as your home of residency. Please fill out this form and submit it to your finance office. Your next LES will reflect this change. Be sure to retain a copy of this form that you submitted. Note: you do not have to make this change permanent.

VA Form 26-8937 - Verification of Indebtedness

On all VA Loans, the mortgage company must send this form to the VA in Houston to verify your status. Please download this form, print and fill it out, and fax it back to me at 281-558-6860 if we will be doing a VA Loan for you.
